Refer a Patient to Valley Children's

Thank you for choosing Valley Children's as a trusted partner in your patients' care. Our experienced staff and specialists will work hand-in-hand with you to ensure that your patients are provided the highest quality care.

For your convenience, there are several ways to refer a patient to Valley Children's pediatric specialists or maternal-fetal medicine specialists:
 

Refer a Patient to a Pediatric Specialist

CareLink: Refer a patient via Valley Children's CareLink portal. By signing up for CareLink, you will be able to access patient information, follow the progress of care for referred patients and more.

eReferral: Refer a patient to Valley Children's using our convenient eReferral system. You will receive a confirmation email once your referral has been submitted.

Fax: PDF Referral Form can be downloaded and faxed to Valley Children's.
 

Refer a Patient to the Maternal Fetal Center

The Maternal Fetal - Prenatal Diagnostic Physician Referral Form can be filled out and faxed to Valley Children's Hospital at 559-353-6710. Please note that we need the Patient Prenatal Records. Please fax the records along with the referral to 559-353-6710.
 

For Urgent Referrals

The urgent flag should be used solely for patients requiring a consultation within a week. For emergent referrals, please contact the Access Center at 866-353-KIDS (5437) to speak directly with a physician or send the patient to the Emergency Department for immediate treatment.

Valley Children's has a one-call solution dedicated to referring providers and their staff. To contact the Valley Children's Provider Assistance Line, call 559-353-8800.

To arrange transfer/transport to Valley Children’s Hospital or if you are a provider needing to immediately speak to a specialist, call the Access Center at 866-353-KIDS (5437).
